If you are a patient requesting medical records, all; Operative Notes, Endoscopy Reports, Pathology, Labs and Imaging Reports can be obtained through Mercy Medical Center Mount Shasta's Health Information Department (530) 926-9341.
All in-office and procedure notes are forwarded from our office to your Primary Care Provider.
If you find that you still need to contact us for records, please use the contact tab at the top of the page or leave a message on 530 918 9331. Please provide your name, date of birth, what records you require and a physical mailing address (Fax copies will not be available).
